ApplicationForms Bot

Description

ApplicationForms is a Discord bot and accompanying dashboard that allows you to create and manage application forms for your community. Submissions are saved directly to the ApplicationForms dashboard, and the bot also sends notifications to your Discord server so moderators can quickly review responses. The bot is designed to be easy to use and flexible.


Commands

Server Commands:

  • /blacklist - Blacklist a user from using submitting forms

User Commands:

  • /help - View this help message
  • /myapplications - View all the applications that you have submitted

Application Form


Dashboard

When you visit the ApplicationForms Dashboard, you’ll see a list of all the Discord servers where you have moderator or admin permissions. Once inside a guild, you can view all the forms you’ve created for that server, as well as submission data and form settings. The dashboard provides a centralized way to create, edit, and manage forms while keeping everything organized.


Configurations

Within the form configuration settings, you can customize the following options to tailor your application:

  1. Title: Set a descriptive and engaging title for your form.
  2. Description: Provide context or instructions for users filling out the application.
  3. Image Banner URL: Add a visual touch by providing a URL for an image banner that will display at the top of the form. We strongly recommend using a 1600x400 image, as any other size will be automatically resized to fit these dimensions.
  4. Multiple Submissions: Decide if users can submit multiple applications. If enabled, there’s a 6-hour cooldown between submissions.
  5. Response Channel ID: Set the ID of the Discord channel where submitted applications will be sent. Ensure the bot has permission to read and write in this channel.
  6. Open/Close Applications: Control whether the form is open or closed for submissions.

Fields

When configuring your form, you can create and edit the questions users will answer. There are four types of fields available:

  1. Short Text: A single-line input field for brief answers, limited to 500 characters.
  2. Long Text: A multi-line input field for more detailed answers, limited to 10,000 characters.
  3. Select (Radio Options): Allows users to choose one option from a list, with a maximum of 10 options.
  4. Checkbox (Multiple Options): Lets users select multiple options from a list, also with a maximum of 10 options.

Responses

In the Responses section, you can view all user submissions in one place. Each submission provides all the answers from the applicant, along with management options. If a submission has been reviewed or is no longer relevant, you can archive it to keep your active submissions organized. If you encounter inappropriate or troll submissions, you have the option to report them to our team for review and action.

All archived submissions are stored in the Archives section. No submissions are ever deleted, ensuring that you can always retrieve them if needed. If a submission requires further attention or revision, you can easily unarchive it and move it back to the active responses list. This system ensures that submissions are always accessible and manageable, even after they’ve been archived.
